Pathways to Benefits

A Phronesis initiative for people with disabilities and their families

Many people with disabilities miss out on benefits they may qualify for. The forms are confusing, the rules are hard to follow, and no one explains where to start.
Phronesis Community Hope Foundation is launching Pathways to Benefits to help change that. We are just getting started, and we are building a free, caring, one-on-one service over time.

Right now: we connect you

While we grow this initiative, we refer clients to trusted partner organizations and services that can help with benefits applications and related supports. Contact us and we’ll listen to your situation, point you to the right place, and help you take the first step.

What we hope to help with

  1. Understanding which benefits you may qualify for, such as the Disability Tax Credit, the Canada Disability Benefit, and Ontario disability programs
  2. Gathering documents and completing applications
  3. Connecting you to services for health, legal, housing, food, counselling, and settlement support
  4. Following up so you don’t have to navigate alone

Who it’s for

Adults and seniors with disabilities, family members and caregivers, newcomers, survivors of abuse, and anyone facing barriers to support.
Growing together

We are building partnerships with shelters, newcomer agencies, health providers, community centres, and cultural and faith organizations so help reaches people where they feel safe. Want to refer someone or work with us? Get in touch.

Our support is free, confidential, and respectful of your language, culture, and story. We cannot guarantee a benefit decision, but we can make sure you don’t have to find your way alone.
